Functional Skills (4748) Level 1 and Level 2 Exam change notification form

Centres must notify City & Guilds of any exceptional changes to the date, time and/or location of scheduled bookings for 4748 paper and on-screen (e-volve) exams. Notification must be provided at least one working day prior to the scheduled exam date or the revised exam date, whichever is earlier.
